Your child must be 5 years of age by September Ist of this year to register. On April 4th, please bring your child for a quick screener, along with the proper paperwork listed below. Use the QR below to fill out the online registration paperwork prior to coming.
Your child will not be actively registered without all forms turned in.

- Immunization records
- Oral Health Screening
- Hearing & Vision Test Results
- State Issued Birth Certificate - not the hospital one
- Proof of Residency
